This is an exciting opportunity to help shape how communities, charities and health services work together across Hillingdon. You'll play a key role in connecting local organisations with NHS neighbourhood teams, ensuring community voices influence decisions and helping to tackle health inequalities through innovative, community-led approaches.

Building relationships with local charities, community groups and NHS partners

Mapping community assets and identifying opportunities for collaboration

Supporting prevention projects that improve health and wellbeing

Helping local organisations understand and engage with neighbourhood working

What We're Looking For
