We are thrilled to announce an exciting opportunity to join our dynamic team Wellbeing Service as a Wellbeing Support Advisor.

As a Wellbeing Support Advisor, you will be an integral member of the new Care Connection Teams (CCT) based in one of the three Neighbourhood Hubs in the Hillingdon Borough. You will work alongside GPs, Guided Care Matrons, a Senior Administrator Coordinator, Health Care Support Workers, and other team members to support residents with health, wellbeing, and social prescription services.
Main Responsibilities:
• Manage a caseload of clients with long-term health conditions and social isolation.
• Conduct assessments and develop support plans.
• Provide holistic, person-centred support and social prescription.
• Advocate for clients and guide them through services.
• Collaborate with health professionals and promote the service.
• Maintain records and contribute to service evaluation.
Requirements:
• Experience in a similar role within health, social care, or the voluntary sector.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a multidisciplinary team.
• Commitment to personal and professional development.
