About the Role
About The Service:
Our High Intensity User (HIU) service is dedicated to providing tailored support for individuals who frequently use Urgent & Emergency Care (UEC) services. Our goal for this service is to improve health outcomes and reduce inequalities by ensuring HIU clients receive the most appropriate medical and psycho-social support.
We are looking for a dedicated and passionate Wellbeing Support Officer to join the HIU service.
The Role
As the HIU Wellbeing Support Officer, you will play a crucial role in managing and supporting a caseload of clients with diverse and complex needs.
Your responsibilities will include:
• Caseload Management: Directly manage a caseload of HIU clients, aiming to reduce emergency admissions and unscheduled care interactions through tailored support.
• Client Engagement: Work with clients facing challenges such as mental health issues, addiction, homelessness, and more. Provide emotional and practical support to help them navigate the health and care system.
• Service Coordination: Facilitate access to relevant agencies and services, advocate for clients, and provide moral support at appointments and court hearings.
• Safeguarding and Risk Management: Be vigilant about safeguarding issues and report concerns to relevant agencies and leads. Maintain timely and accurate records of client interactions.
Key Responsibilities
• Health Coaching and Motivational Interviewing: Use these techniques to manage clients with complex needs, conduct outcome assessments, and develop individualised action plans.
• Community Support: Provide intensive floating and emotional support to improve clients' wellbeing and self-management.
• Advocacy and Accompaniment: Support clients at appointments, advocate on their behalf, and ensure they engage with relevant services.
• Multi-Disciplinary Meetings (MDTs): Attend weekly MDTs with various stakeholders to update on client cases and inform ongoing work.
• Client Empowerment: Encourage clients to become active partners in their own health and wellbeing.

Requirements
• Experience: Previous experience in a similar role within health or social care.
• Skills: Strong organisational, communication, and interpersonal skills.
• Knowledge: Understanding person centred care, health and social care systems and the needs of vulnerable populations.
