About the Role
About The Service:
Our High Intensity User (HIU) service is dedicated to providing tailored support for individuals who frequently use Urgent & Emergency Care (UEC) services. Our goal for this service is to improve health outcomes and reduce inequalities by ensuring HIU clients receive the most appropriate medical and psycho-social support.
We are looking for a passionate and dedicated HIU Care Coordinator.
The Role
As the HIU Care Coordinator, you will be central to coordinating and identifying individuals for the service.
Your responsibilities will include:
• Identifying and Supporting HIU: Work with local teams to identify HIU clients, review their needs, and refer them to suitable providers.
• Coordinating Care: Facilitate discussions between primary care, hospital, and community services to ensure seamless care for HIU clients.
• Publicising the Service: Promote the HIU service to users and staff across the Borough, increasing visibility and awareness.
• Managing Caseloads: Track patient activity, maintain accurate records, and manage patient caseloads to deliver measurable outcomes.
• Liaising with Partners: Build and maintain relationships with health and social care professionals, third sector organisations, and other stakeholders.
Key Responsibilities
• Service Delivery: Act as the central point of contact for all referrals, provide information and guidance, and update urgent care plans as needed.
• Service Set-Up: Implement referral and feedback mechanisms, develop a directory of support services, and ensure information is up-to-date.
• Communication: Develop materials, attend events, and provide briefing sessions to promote the service.
• Tracking and Reporting: Use the HIU dashboard to track processes and outcomes, and maintain accurate records on the HIU Patient Case Tracker.

Requirements
• Experience: Previous experience in a similar role within health or social care.
• Skills: Strong organisational, communication, and interpersonal skills.
• Knowledge: Understanding of the health and social care system and the needs of vulnerable populations.
