Wellbeing Support Officers

Various vacancies available

Salary: £22,100 - £24,450 p.a.

Hours: 35 hours per week

Contract: Various Permanent

This role is based within the Wellbeing team, who work in partnership with Hillingdon GP surgeries, to support adults to better manage long term health conditions, frailty, social isolation and to bring people into greater contact with their local community.

 

Wellbeing Support Officers will work with the wider voluntary & statutory sectors to support Hillingdon residents. The post holders will proactively manage a case load of clients to empower them to ‘self-manage’ and find their own solutions to non-medical problems. They will undertake client assessments, including visiting clients in their home, provide information and advice along with practical and emotional support and inputting into care planning.  

 

 

Wellbeing Support Advisors

Hours: 35 hours per week

Contract: Various Permanent - Plus 1x Fixed Term (Maternity Cover)

This role is an integral part of the expanded Care Connection Team model of population-based care which has an emphasis on prevention by supporting the 2% of the adult population most at risk of non-elective episodes/admissions. 

 

Wellbeing Support Advisors will deliver a range of wellbeing and social prescription services to people most at risk of developing escalating care needs who are aged 18 and over, registered with a Hillingdon GP. The post holders will be expected to manage a caseload of clients through assessment, support planning, interventions and onward-referral and seek to appropriately reduce the number of emergency admissions to hospital, A&E visits, ambulance calls and unscheduled care interactions for them. 

The post holders will contribute to the overall operation of the H4All Wellbeing Service, maintain a personal comprehensive knowledge of the range of support services available through the wider Hillingdon voluntary and community sector and work with the H4All Wellbeing team and other appropriate partners to identify culturally specific support, for example within:  BAME and refugee communities; diverse faith and cultural groups and LGBTQ+ communities.
