Full time - 35 hours

 

H4All are looking to recruit Wellbeing Support Advisors, to help deliver the Neighbourhood model of population-based care. This integrated model of preventative care aims to support the 15% of the adult population most at risk of non-elective episodes/admissions.

The post holders will be an integral part of the new expanded H4All Wellbeing Service Neighbourhood Teams that will deliver a range of health, wellbeing and social prescription services to people aged 18 and over who are registered with a Hillingdon GP or are resident in the borough of Hillingdon.

The successful candidates will manage a caseload of clients through assessment, support planning, and onward-referral and seek to appropriately reduce the number of: emergency admissions to hospital, A&E visits, ambulance calls and unscheduled care interactions for them.

High Intensity User Service: Wellbeing Support Officer

£26,500 - £29,500 per annum (depending on experience)

Full time - 35 hours (with flexible working)

 

Hillingdon HIU service, delivered by H4All, is a social prescription based, non-clinical service, supporting patients to reduce unnecessary attendance and admittance at Accident & Emergency (A&E) for the top 50 patients with acutely high usage of Hillingdon A&E.

 

Utilising a Health Coaching based model of support our HIU Wellbeing Support Officers provide motivational interventions and ongoing emotional support to empower patients to:

  • better understand and address the causes of their ill health

  • better self-manage their conditions

  • improve their quality of life

  • utilise alternative solutions other than attending A&E

  • access services within the community

 

The HIU Wellbeing Support Officers operate in a flexible and holistic way and work closely with Hillingdon Hospital’s A&E team and the existing Multi-Disciplinary team including London Ambulance Services, Adult Social Care, CMHTs, GPs, the Metropolitan Police, and wider third sector organisations to support these top 50 patients.

MyHealth Administration Assistant

MyHealth Hillingdon is an education and empowerment programme, offering patients and residents of Hillingdon long term condition workshops to help them manage their health and take back control.

We also offer workshops for signposting around NHS services and first aid training for new parents.

 

The aim of MyHealth is to give patients that knowledge and confidence to feel like they have complete control of their health, to then reduce A&E admissions for long term conditions, Unnecessary GP appointments and misuse of NHS services.

 

Main purpose of the role:

The post holder will be an integral member of the MyHealth Hillingdon team, that deliver a range of health & wellbeing workshops across the London borough of Hillingdon.

The postholder will be supporting the programme coordinator with daily tasks and the day to day running of the programme. The role involves working across a number of different systems with varied engagement, helping to maintain databases, speaking with patients by telephone and working with the team to promote our brand through social media and community outreach.

H4All is being widely recognised for its innovation and new ways of working. H4All combines the expertise of its member organisations and the wider voluntary sector to bring health benefits to Hillingdon residents. 

H4All CIO is a partner in the local Hillingdon Health and Care Partnership (HHCP) which includes the Hillingdon Primary Care Confederation as well as the local NHS foundation trusts providing community care services and acute hospital care.

Harlington Hospice have a new role for an enthusiastic.

 

Payroll & Pensions Administrator

Salary: £15,700 per annum (based on £28,000 FTE)

Hours: 21 hours per week, flexible between Monday to Friday.

 

Location: Harlington Hospice, Key House, Yiewsley, West Drayton. Some home working may be available by agreement.

Contract Type: Permanent following 6 months probation

Responsible To: Finance Manager

 

The role is responsible for managing, maintaining and recording accurate, relevant, financial information for all payroll and pension related items. Payroll information comes from a staff time planning and monitoring system – Webroster - which is uploaded on a monthly basis into the Sage payroll database. Training on this system will be provided. The Hospice offers two pension schemes – a Scottish Widows money purchase scheme and the NHS final salary pension scheme for clinical staff.

 

 

Required Qualifications and Experience

 

  • Minimum of 2 Years working within a Payroll Department operating a payroll of more than 100 employees

  • Working knowledge of UK tax and NI calculations, statutory payments, pensions and year ending reporting

  • Experience using Sage 50 Cloud Payroll

 

Desirable Requirements

 

  • Minimum of 1 Year Customer Service experience

  • Intermediate Microsoft Excel skills

  • ACCA Part or Full Qualified or CIMA Part or Full Qualified

  • Experience of running payroll in the NHS or for a domiciliary care provider
