We are seeking motivated and passionate candidates for the following role:

 

High Intensity User Service: Wellbeing Support Officer

£26,500 - £29,500 per annum (depending on experience)

+ contributory pension

Full time - 35 hours (with flexible working)

 

Hillingdon HIU service, delivered by H4All, is a social prescription based, non-clinical service, supporting patients to reduce unnecessary attendance and admittance at Accident & Emergency (A&E) for the top 50 patients with acutely high usage of Hillingdon A&E.

 

Utilising a Health Coaching based model of support our HIU Wellbeing Support Officers provide motivational interventions and ongoing emotional support to empower patients to:

  • better understand and address the causes of their ill health

  • better self-manage their conditions

  • improve their quality of life

  • utilise alternative solutions other than attending A&E

  • access services within the community

 

The HIU Wellbeing Support Officers operate in a flexible and holistic way and work closely with Hillingdon Hospital’s A&E team and the existing Multi-Disciplinary team including London Ambulance Services, Adult Social Care, CMHTs, GPs, the Metropolitan Police, and wider third sector organisations to support these top 50 patients.

We are seeking motivated and passionate candidates for the following role:

 

Wellbeing Support Advisor

£23,000 - £25,500 per annum (depending on experience)

+ contributory pension

Full time - 35 hours

 

H4All are looking to recruit a Wellbeing Support Advisor, to help deliver the Neighbourhood model of population-based care. This integrated model of preventative care aims to support the 15% of the adult population most at risk of non-elective episodes/admissions.

The post holders will be an integral part of the new expanded H4All Wellbeing Service Neighbourhood Teams that will deliver a range of health, wellbeing and social prescription services to people aged 18 and over who are registered with a Hillingdon GP or are resident in the borough of Hillingdon.

The successful candidates will manage a caseload of clients through assessment, support planning, and onward-referral and seek to appropriately reduce the number of: emergency admissions to hospital, A&E visits, ambulance calls and unscheduled care interactions for them.

H4All is a formal joint venture between five voluntary sector organisations in the London Borough of Hillingdon that is being widely recognised for its innovation and new ways of working. H4All combines the expertise of its member organisations - and works with the wider voluntary sector - to bring health benefits to Hillingdon residents.

H4All CIO is one of the pillars of the local Integrated Care Partnership, Hillingdon Health and Care Partnership (HHCP), which includes the Hillingdon Primary Care Confederation as well as two local NHS Foundation Trusts, which provide community care services and acute hospital care.

H4All works with a broad range of partners in North West London, H4All has driven the development of 3ST (Third Sector Together) NWL. 3ST is a group of like-minded larger charities that have been working together since 2019 across the eight boroughs of NW London: Harrow, Hillingdon, Brent, Hounslow, Ealing, Hammersmith and Fulham, Kensington and Chelsea and Westminster.

 

We are looking for an Independent Chair to:

  • To support and shape the development of H4All future strategic vision

  • To Chair bi-monthly H4All Board meetings and any extraordinary meetings as required

  • To influence at a senior level in partner and stakeholder organisations

  • To champion and advocate the needs and views of the local community
