Lung Health Engagement Worker (Hillingdon)
Salary: £25,480 pa
Hours: 35 hours per week, Monday to Friday
Based at: Community based work across the London borough of Hillingdon and at the H4All head office, Key House, 196 High street Yiewsley, UB7 7BQ (flexible and home working options are available to all staff subject to negotiation and service need)

Three Age UK brand partners, Age UK Hillingdon, Age UK Hammersmith and Fulham and Age UK Sutton are working in collaboration with the West London Cancer Alliance on a Target Lung Programme Partners to deliver a coordinated and high-quality service to support residents' access and engagement with their Lung Health screening programme.
West London Cancer Alliance, through Royal Brompton Hospital, have developed an early diagnosis project that aims to diagnose patients with lung cancer earlier by identifying the population at increased risk, inviting them for a lung health check and where eligible, a low dose CT (LDCT) scan.
The project aims to increase awareness and uptake of the lung health checks by empowering local people to take control of their lung health by increasing participation in targeted lung health checks by those aged 55-75 who are smokers or ex-smokers and increase awareness of the higher risk of lung cancer for this group and the benefit of attending the lung health check to intervene early.
The post holder will:
Work in collaboration with our Target Lung Programme Partners to deliver a coordinated and high-quality service to support residents' access and engagement with the screening programme.
Empower local people to take control of their lung health by:
Communicating and actively delivering the key messages of the Targeted Lung Health Programme to local residents through a programme of roadshows, events, talks and public engagement.
Co-producing information materials to inform and advise local residents and the wider third sector.
Contributing to the education of the wider health and wellbeing alliance network, including community health champions' network.
Supporting identified clients with barriers to accessing the screening programme as appropriate.
Identifying wrap around support for those who need it following Lung Health screening.
Contribute to the development of the service.

Social Prescribing Programme Manager
£32,000 - £36,000 p.a. plus contributory pension
Full-time: 37.5 hours
The Social Prescribing Project Manager will work in collaboration with Hounslow Consortium (GP Federation) and the Clinical Lead in each PCN to deliver a coordinated and high-quality Social Prescribing Link Worker service that delivers a range of wellbeing and social prescription services within Hounslow PCNs – supporting residents to better self-manage and engage with the
extensive range of support in the community.
The Social Prescribing Programme Manager will support the development of Social Prescribing in Hounslow, represent and promote the service to stakeholders, develop integrated ways of working with providers and monitor and evaluate the effectiveness of the service against agreed targets.
The Link Worker programme is a national initiative from NHS England and NHS Improvement, and we are seeking an individual with a background in health, care, public health or voluntary sector services.
